The Therapy Sessions & Progress Tracker in Google Sheets is a one-page session log for therapists, counsellors and coaches who want a clean record of who came, what was worked on and how the score moved. Every session takes 16 columns, 3 of them fill themselves, five pick-lists come from a single List tab, an Average Progress Score card sits at the top left and 4 charts redraw as you type. Fifteen worked sample sessions dated 03-Aug-2026 to 21-Aug-2026 ship inside it, so nothing is blank the first time you open it. It is a one-time purchase at $6.99 rather than a monthly practice-management subscription, the copy lands in your own Google Drive, and you can edit it, export it or delete it whenever you like.

Please read this before buying. This is a Google Sheets spreadsheet you type into and that you control. It is not clinical software, not a medical device, not an EHR or clinical record system, and not a substitute for professional judgement, supervision or your own case notes. It does not assess, diagnose or treat anything. The Progress Band column is an ordinary formula that groups the Progress Score you typed – “On Track” means a formula matched your own number, nothing more, and it is not a clinical outcome measure. The file has no encryption, no password protection, no access log and no audit trail, so it is not HIPAA-, GDPR- or PIPEDA-compliant storage and it carries no compliance certification of any kind. If you enter any client information, you are the data controller: how it is stored, who you share the sheet with, whether you use initials or codes instead of full names, and how it fits your practice’s confidentiality rules are all your responsibility. The template’s own How to Use tab states the same thing.

Key Features of the Therapy Sessions & Progress Tracker in Google Sheets

  • 16 columns per session – Session ID, Session Date, Client Name, Therapist, Therapy Type, Session Mode, Duration (Min), Attendance, Progress Score, Mood Rating, Homework Status, Goal Focus, Session Fee, Progress %, Progress Band and Session Notes.
  • 3 self-filling columns – Session ID numbers each row, Progress % converts the score, and Progress Band returns On Track, Progressing, Needs Support, Pending or Missed. All three stay blank until you type a client name, so an empty sheet shows no errors.
  • 5 dropdowns from one List tab – Therapy Type, Session Mode, Attendance, Homework Status and Goal Focus all read the List tab, rows 2 to 500, so you edit one place and every pick-list follows.
  • Dropdowns grow automatically – add your own modality or goal to the List tab and it appears in the dropdown straight away, with no trip through Data validation.
  • Average Progress Score card – one headline number with a Sessions Logged count underneath, recalculated on every entry. The sample data shows 6.7 across 15 sessions.
  • 4 live charts – Session Count by Therapy Type, Session Count by Attendance, Progress Score by Session Date, and Progress and Mood Rating by Session Date.
  • No hidden helper tables – all four charts read the data columns directly from rows 16 to 1000, so a new session appears in the visuals immediately and there is nothing behind the scenes to break.
  • Missed sessions do not distort the average – leave Progress Score and Mood Rating blank on a no-show or a cancellation and the headline number ignores them.
  • Colour-coded columns – therapy types, attendance, homework status and progress bands each carry their own shading, so a month of rows has a shape you can read at a glance.
  • 15 worked sample sessions – three weeks of realistic entries across eight modalities. Delete them when you start your own.
  • 4 tabs only – Therapy Sessions Tracker, List, How to Use and Explore More Templates. There is nowhere to get lost.

What’s Inside the Therapy Sessions & Progress Tracker in Google Sheets

Therapy Sessions Tracker. The single working page. The Average Progress Score card sits top left, the four charts run across the top, and the session table fills the rest. You type left to right: the date, who the session was with, which therapist ran it, the modality, whether it was In-Person, a Video Call, a Group Room or a Home Visit, how long it ran, and whether the client Attended, was a Late Arrival, a No-Show or Cancelled. Then the two 1-to-10 numbers – Progress Score and Mood Rating – followed by Homework Status, the Goal Focus for the session, the Session Fee, and a short note. Session ID, Progress % and Progress Band fill in behind you.

List. The vocabulary tab. The sample file ships with eight therapy types (CBT, DBT, Couples Therapy, Family Therapy, Group Therapy, Play Therapy, Speech Therapy and Trauma / EMDR), four session modes, four attendance states, four homework states and a set of goal focuses such as Anxiety Management, Trauma Recovery, Social Skills, Anger Management, Stress & Coping, Communication and Speech & Language. Replace any of them with the words your practice actually uses.

How to Use. Written on the sheet itself: five numbered steps plus notes on turning the dropdowns into coloured chips, how the charts are wired, where the colour palette lives, how to switch Session Fee to another currency, and a plain privacy statement that this is an organisational tool and not a clinical record system.

The honest summary: practice software books appointments, stores protected clinical notes on certified infrastructure and sends the invoices. This does none of that, and it is not trying to. What it gives you is an attendance and progress record in a plain grid you can sort, filter, chart and hand over, with no seat licence and no renewal date.

Who This Template Is For – and Who It’s Not For

It is for a solo therapist, counsellor or coach who wants attendance and progress in one place without paying per seat; a small practice tracking session counts, modality mix and no-show rates across two or three clinicians; a supervisor reviewing how a caseload is moving before a supervision hour; a speech, play or occupational therapist logging goal focus session by session; and a student or trainee keeping a practice log with initials or client codes rather than names.

It is not for storing clinical notes or protected health information – it has no encryption, no access control beyond Google Drive sharing, no audit trail and no compliance certification. It is not a scheduling system: it sends no reminders, no confirmations and no emails, and nothing happens unless you open it. It is not billing software – Session Fee is a number you type, not an invoice, a claim or a superbill. And it is not an outcome-measurement instrument: it contains no validated scale such as PHQ-9, GAD-7 or ORS/SRS, only the 1-to-10 numbers you choose to enter.

How to Use the Therapy Sessions & Progress Tracker in Google Sheets

  1. Open the PDF in your download and click the Google Sheets copy link. A private, editable copy appears in your own Drive.
  2. Decide first how you will identify clients. Initials or a client code keeps the sheet far safer than full names, and every column downstream works the same way.
  3. Open the List tab and replace the sample therapy types, session modes, homework states and goal focuses with the vocabulary your practice uses.
  4. Go to the Therapy Sessions Tracker tab, delete the 15 sample rows (rows 16 to 30), and log your first session.
  5. Leave Session ID, Progress % and Progress Band alone – they are formulas, and they stay blank until a client name is entered.
  6. Leave Progress Score and Mood Rating empty for a no-show or a cancelled session so they never drag the average down.
  7. After a month, read the four charts together: which modalities dominate, how much of the caseload is Late Arrival or No-Show, how the progress line moves, and whether mood tracks progress on the same dates.
  8. Optional, ten seconds: select column E (then F, H, K and L), open Data > Data validation, click the rule, and set Display style to Chip to turn the dropdowns into coloured pills.

Real-World Use Cases

Nadia, private practice CBT therapist. She sees twelve clients a week and wanted one view of whether progress scores were actually moving. Two months in, the Progress Score by Session Date line answers it, and clients are identified by initials only – the sheet never holds a full name or a note she would not read aloud.

A three-clinician counselling room. The Therapist column separates the caseloads, the Session Count by Attendance donut shows how many slots are lost to no-shows each month, and Session Fee gives a rough revenue picture before it goes anywhere near the accounts. Clinical notes stay in their practice system; this sheet is the operational layer on top.

Priya, school speech therapist. Goal Focus is her key column – Speech & Language, Social Skills, Communication – and Homework Status tells her which pupils practised between sessions. She shares a filtered view-only copy with the SENCO before each review meeting.

Frequently Asked Questions

Is this clinical software, an EHR, or a diagnostic tool?

No, to all three. It is a Google Sheets spreadsheet you type into. It does not assess, diagnose or treat anything, it gives no clinical advice, and it does not replace professional judgement, supervision or your own case notes. The Progress Band column is a plain formula grouping the score you entered – “On Track” means a formula matched your own number, nothing more.

Is it HIPAA or GDPR compliant?

No. It has no encryption, no password protection, no access log and no audit trail, and it carries no compliance certification of any kind. It is not appropriate storage for protected health information or clinical records. Your copy sits in your own Google Drive under whatever sharing settings you apply, and you are responsible for what you put in it and who can see it.

Can I use it with real client names?

That is your decision and your responsibility as the data controller. Many buyers use initials or a client code instead, which keeps the sheet useful for counting sessions and reading trends without holding identifiable information. Whatever you choose, check it against your practice’s confidentiality policy and your local data-protection rules first.

Will it send appointment reminders or invoices?

No. It sends no notifications, emails or alerts, and Session Fee is simply a number you type. Nothing happens unless you open the sheet.

What exactly do I get?

A PDF containing a Google Sheets copy link. Click it and a fully editable copy lands in your own Drive. There is no add-on to install and no account to create beyond a free Google account.

Can I change the therapy types and goal focuses?

Yes, and that is the intended first step. Type your own into the List tab, rows 2 to 500, and the dropdowns pick them up straight away.

How many sessions can it hold, and does it work on a phone?

The charts are wired to rows 16 to 1000 out of the box, and it is an ordinary Google Sheet so you can extend beyond that. It works on the Google Sheets mobile app – typing a session is comfortable, though the four charts read better on a larger screen.
