This is a business reporting template for a home care agency’s visit log. It summarises visits, schedule reliability, travel burden, payer mix and billed value. It is not a care-planning, scheduling, rostering, electronic visit verification, medication or safeguarding system, and it makes no clinical, diagnostic, treatment-outcome, care-quality or patient-safety judgement. “Patient Acuity” and “Readmission” are two columns you fill in yourself; the dashboard counts them, it does not assess them.

Read this before you paste real data. The Data sheet has 17 columns, and two of them hold people’s names: Patient Name (45 fictional patients in the sample) and Caregiver (14 fictional staff). The Patients page charts billed value by patient name, and the Search page prints a named patient record. There are no home addresses, postcodes, GPS coordinates, phone numbers, dates of birth, diagnoses, medications or clinical notes anywhere in the file — Region is a broad service-area label (Central Metro, East Valley, North County, Riverside, South Ridge, West Hills) and Travel Miles is a distance number, not a route or a location. But if you paste real patient names into that column, you have put identifiable health information into an ordinary Google Sheet, protected by nothing more than your Drive sharing settings. Use an internal patient code instead. Every chart, pivot and the Search lookup work identically with codes, because nothing in the build keys off a name format. There is no live connection to any EHR, billing platform or scheduling system, and no compliance certification of any kind is claimed or implied.
